The NEW AFLAC GROUP COVERAGE FOR LOCAL 2068 IS NOW IN FORCE! (Effective 10/1/18)
Here are instructions and some helpful tips for filing your Aflac Group claims:
1. By now, you should have received a policy certification in the postal mail that contains the group number and your personal certificate number. You will need this number when submitting claim forms. If you have not received it yet, please email our Aflac Lady at This email address is being protected from spambots. 2. Login to the union's website and click on Member Links to access the claims forms, or go directly to: https://aflacgroupinsurance.com/customer_service/file_claims.aspx
3. Download one of the following four claims forms, which corresponds to your medical issue:
o Accident Claim Form - for acute injuries occurring on or after 10/1/2018
o Hospital Claim Form - for hospital admission or in/out surgeries occurring on or after 10/1/2018
o Critical Illness Claim Form - for specific illnesses that are covered (cardiac, cancer, etc.) diagnosed on or after 10/1/2018
o Health Screening or Wellness Benefit Claim Form - for your annual physicals/cancer screenings each year on or after 10/1/2018
4. Fill out your claim form and direct deposit form, attach the appropriate proof of diagnosis/treatment (see attached), and either fax or email your claim to the following:

§ TIP: Download a scanning app (e.g., TinyScanner) to your cell phone. You can then scan all your pages as a PDF right to your cell and export into email with one function. Easy peasy...claims submitted within 10 minutes!
I've attached an updated copy of the Plan Summaries showing which medical record(s) or proof of treatment is needed to submit for those specific benefits listed. This will make filing and processing your benefits MUCH easier.
