Follow the instructions below to obtain a copy of the Member Profile Form and return it to the Local 2068 office.

1. First download/save a copy of the Member Profile Form.

2.You will be prompted to open the file as “Read Only.” Please accept the prompt.

3. Fill in the appropriate information. HINT: You may use the [Tab] key to move from field to field

4. Save the Member Profile form as a different file name

5. Launch your email program.

6. Email a copy of the file to office@fairfaxfirefighters.org
